This came from the rollbacks, people would glass an item once for 3 lines, get rolled back and get 2.
But are you sure those people didn't get rolled back to the point just before using the potential scroll as well? I had the same drop with hidden potential rolled back 2 times and on all three reveals winded up with the same potential.
