Both are true and false.
Let me elaborate. Success and failure are words that tell whether you managed to achieve a specific action or not. If the action's timespan is out, and you cannot achieve it anymore, then you have either permanently succeeded or failed the action. However, if the action's timespan is not out, then you might have permanently or temporarily succeeded (or not), depending on the wording.
Examples on this would be goals in MapleStory. (Since the set goals is a subset of actions blah blah)
"I was the first person in GMS to level 200" is a permanent achievement done by FangBlade.
"I have been the highest-leveled person in MapleStory" is a permanent achievement done by Tiger in a timespan reaching from GMS started to FangBlade turned 200.
"I was not the first person in GMS to level 200" is a permanent achievement done by everyone but FangBlade (It is turned negatively, but it's an achievement nonetheless).
"I am the strongest Paladin in the MapleStory-world of all time" - Temporarily achievement.
As for the learning-part, every action earns you experience, which means that if you use that experience, you have learnt something. Thus, every single action gives you experience, no matter how little experience it gives you or how little you use of that experience.
