Seems to be something that happens with most multi-hit skills that do more than 3 - sometimes they'll do identical crits and damage on several of the hits.

For example, Soul Driver, if the first hit crits, always shows another crit on the 4th hit (and as you can see, it does the same damage).
Other than that, it's independent - the 2nd, 3rd, and 4th hits can all crit without another. But there's never a first hit crit without a 4th hit crit.
What I'm having trouble determining is whether this translates to actual damage, or if it's just a visual problem.
As for the effect, if it really is:
Well, the first hit effectively deals double crit damage. So instead of 5% chance of 20~50%, it's 5% chance of 40~100% extra.
I've noticed this on my Bandit as well, but not my DrK - and obviously my Paladin doesn't have any 4-hit attacks, so it'll never do this.

Dark Knights don't do this with Crusher - but it's not really a 4 hit skill, is it? They also run a 35% crit by default.

ive noticed this happens in siege mode fairly often, along with noncrits once in a while.
the only common ground that ive noticed is im either blinded or the monster always dies when i see [up to 6 of the same crits] or [any noncrits at all].
pretty weird seeing *11672 6 times in a row against papa pixie or lord pirate

I've noticed this on my Buccaneer as well, and it is pretty strange. I'm doubting that the damage seen is legit, as someone else viewing it may not see the double crits, although I haven't tested it. However with my Bucc, it doesn't always just mimic crits yet regular numbers as well. Usually the first 3-4 hits of demolition mimic the last 3-4 hits of demolition. Really strange.

Happens all the time at grandpa. It's more apparent when there are misses involved.

Yeah, misses get this bigtime. I've seen TT doing all the same damage when bosses cast blindness on my NL.

Shown damage in maple is quite decieving. Most of the time what is shown has nothing to do with the damage dealt.

I've noticed on my DK when I'm in the dojo and I get buffed by the boss, I hit 3 of the same number some times in between misses.

For a Paladin the first 2 crits on Blast are always the same number, and only the 3rd crit hit can be a different number but alot of the time all 3 crit hits are the same number.

To add to that, I've noticed that as long as I'm using blast on a monster in Masteria or Amoria the crits stop acting in sync with each other. I think that this may indicate that the damage isn't actually 100% accurate when it is synced up; however, I've also had people watch my damage and they see the same numbers that I do.

Thought so. It's not just crits, it's any number. Crits, misses, and reg numbers.

There's an upper limit to the amount it will generate (I think it's about 6 per attack). After that, it just loops back to the start of the array. I guess cause generating random numbers is relatively expensive, and they don't want to just keep throwing in more and more.

That may have been just speculation on your part, but it is the best explanation that fit the data we had at that point.

can't remember exactly what happens, but when a DB with shadow partner uses final blow (i think this is the attack) and misses, the 16 hits follow some sort of pattern.
i'll get a screenshot in a while
EDIT: my computer/internet being an idiot, but this is what happens with my DB, when using fatal blow(lolsp) and missing, with one weapon, all the hits from one weapon will be the same, and the other one will be misses, finding a way to upload this right now.
and a weird one:
you can see the first hit was a crit, the next 3 weren't crit, but the next 4 hits were the exact same as the first 4.
