Ultimatums NEVER work. Don't do it. Just straight up tell him what you're going to do and DO IT, them tell him that once he shows that you can trust him again that you'll be more flexible. You're just going to cause yourself to suffer longer both financially and emotionally if you put it off. Being tough is HARD but those feelings of guilt sure beat the hell out of enabling him to be a schmuck.
This is a lesson that will either help him for life or ruin your friendship, and if he relies on you so much than it's not a real friendship anyway. You're going to be the bad guy no matter what. Might as well do it right.

You can change the SSID and Password of the router... or there is a MAC filter that can be used. Just find out what what his PC's Network Card MAC address is, enter it into the Block List, and then enable it. BAM!!! He's denied access to the network and internet.

You can disable the Reset button on most routers to prevent tampering.

It's for security purposes where a router can be used in a business or home type setting, but you don't want it tampered with by users who could tamper with the settings and unblock certain applications, websites, etc.
