One of a Kind Tag only affects looting. Quest might check for the item.
If you have two One of a Kind items, you can equip them both (ie rings)

However, if you trade in Ring 1 for Ring 2 when you hit level 50, and again at 70 with 2 and 3, you only get one at a time.
Lots of room for glitches....
