I can think of two games that are rather popular that have this problem. I don't know if one of those changed, though, since it got an expansion recently. One of them is an 3D MMORPG the other really isn't but still...

Uh, most of the skills were just... pale.
Hitboxes were dumb (dunno if they have fixed that, and even though the first person view helps a lot your enviroment awareness gets cut by more than a half because of it.

Not really. It's just that it's a combination of several different kinds of inertia and forces (including electromagnetic forces from static electricity) that are really fucking difficult to calculate on the fly in real time.
Kind of like a Computational Fluid Dynamics problem but with (thankfully) a lot more constraints. Maybe in another 10 years we will have realistic hair physics in games. I'd say 20 years until it's able to be done by relatively affordable hardware.
