Yep. All buffs expire when you die, so when you die with Soul Stone active it'll expire and you won't be able to use it.
Usually gets fixed with a patch or server check, and usually comes back a few days/week before another patch. Same with familiars attacking but not registering damage, and Magic Crash/Binding Darkness not resisting each other when used consecutively.
