Did shenanigins really get worse?

Is hero mode like a new game+ or just an unlocked difficulty? I want to play the game on the hardest mode possible on the first playthrough.

Hero Mode = take double damage and hearts do not drop AT ALL from ANYTHING (unless you get the Heart Medal, but I count that as cheating).
