The idea of the second movie is actually... quite interesting.
Simply put, it takes place present day and basically shows a man watching the first one and finding it so appealing that he wants to put it in real life practice. We see him fornicate (unfortunately for him AND us, with sand paper) to certain clips from the movie which eventually leads to him raping victims based on the movie and yeah. I think it's some ridiculous number he wants to replicate to make it 100% medically inaccurate.
