This is what I feel is best for Kog'Maw:
-- First six levels, WEWQWR or WQWEWR
-- R > W > Q > E
-- AS specs. Don't go overboard on it though.
-- Boots + pots out of the starting gate.
As usual, what you get depends on the makeup of the other team and what your lane is. Kog'Maw can spec all three damage routes with various degrees of success (AP is the weakest). If you're going to specialize in Living Artillery (AD build), better start on your Manamune early. Poking at 40 MP per shot is just retardedly slow, but land a barrage of good shots, and you have a team at half health before long. Watch your (de)buff icon to see what your cost per shot is currently at. AS build Kog'Maw has THE HIGHEST single-target DPS of ANY traditional ranged carry on average. Ironically I think a fully farmed Tristana or Corki still destroys Kog'Maw though.
Positioning and map awareness will be your friend when playing Koggles, and if you don't have great skill in either, it can hurt you. You could be trying to attack someone from far away, only to have another enemy champ "sneak" up from your flank and kill you because you weren't watching. This is especially true in jungle warfare. If you can peg someone with R, you can auto-attack them around various corners and beyond walls. Also keep in mind that, with exception to E, anything you do to attack will give away your position and possibly your teammates (because you're next to them). In the case of R, if you hit someone, they will be able to see you.

Sometimes offense is your best defense. I know this situation doesn't do what I just said any justice, but one time I was running away from a losing fight, and Ezreal + Kassadin managed to catch up to me. I didn't think I would live because of the slow, so I decided to attack one of them instead. I killed Kassadin before they could kill me (I was actually trying to target Ezreal, but I didn't want to waste time clicking).
In certain other situations, I was able to punish over-zealous melee champs by chucking E at them, then proceeding to DPS them down. This requires anticipation. It's not as good for running away, but sometimes it makes a difference between their successful tower dive and getting away with it, and you getting revenge.
