Ugh I just checked through my replays and I don't have any of my good Irelia games recorded to show you
If you've farmed a lot by the time you have Triforce and GA built you'll be pretty bulky and survive a while in teamfights. If I know my team will probably win the teamfight I'll run in with my ult and hit them all and then proceed to Q the carry with W activated and E (like you said). Autoattack and use other skills whenever possible. If someone gets low immediately Q to them to try to finish/slow/stun them but don't chase and leave your team behind still fighting.
If I'm going to get dangerously low during the fight I save my ultimate and use it during the fight when I get low, or when the other team starts running (I've killed tons of people by just throwing my ult at them while they try to run).
I think Corn had the same problem as you, he wasn't getting as many kills and tons of assists. I don't usually run into that problem but I don't think what I do is really different.
I'll play Irelia tomorrow/tonight and remember to record and I'll upload any good matches. Hope this helped somewhat
