Have you played DFO? Dragon Nest operates kind of like that - mostly instance based, with adjustable difficulties. That seems to be the recent trend; making the overworld simply a quest/trade/chat hub and funneling everyone into instances for actual gameplay. It definitely limits their creativity as far as questing goes.
There's hardly any story, it's like Maple (and the majority of asian MMOs) in that respect. Oh, and it doesn't require that much play time to level cap, but you're hindered by the fatigue system.
