It's a tough call. At least, for me it is. Generally I would like to stay out of the fire as my primary defense, and rely on lifesteal mid-late game.
If you're up against a harass nut during laning, you can opt to get defense then, or kill the guy and make him pay for his insolence.
If you're dying from a single combo that isn't even hard to pull off, you either tower hug or opt for a lane switch.
