Casual lunch or all-out dinner for a first date?

Ask her out towards the end of the project if you want to go for non-casual date type dinner. That way if she rejects you (not saying she will. you got this!), there won't be the awkwardness when you work together on your project that may or may not interfere with your grade.

But if you're in a hurry, ask for lunch. though it may not be the date type environment you want, it can be a better way to "take your minds off things (in this case the project and such. Who wants to talk about school on a date?)".

That's one of the things that stopped me from asking her earlier, the awkwardness if she says no.
But yeah, I just want to treat her to a good time, cause she's usually so busy and whenever we've met up, she always seems tired and/or distracted by her other classes...

Casual lunch all the way. This way you split the bill.

I'd say a late lunch or early dinner. Best time of the day.
You should ask her as soon as possible. You said yourself that you feel like you wasted enough opportunities.
Remember, she gave you her number in the first place so she must like you at least enough for her to want you to call her.

Well, I've attempted to ask her out for lunch like 2-3 times already but she was either too busy that day, or had practice at the same time. (She plays badminton)...so I wouldn't say I've missed opportunities, but more like, used them to no avail, I guess.
And well, she gave me her number 'cause she wanted my help for a programming class and she didn't know how to contact me besides phone. I wouldn't really say that she gave it because she liked me and just wanted me to call. >_<
