The hardest instance dungeons of any game are in AIKA, mainly due to glitchy/horrible coding requiring obscene amounts of player skill and knowledge of all the areas where you might randomly glitch into the wall and pull 500 enraged gods upon your feeble party, to compensate. In the end, very few games are "difficult", they are simply something to do in a group in a semi-enjoyable fashion. B&S won't be a "difficult" game either, it will be challenging in that you need to memorize each enemy's individual attack pattern which in turn gives you a sense of satisfaction, but not "difficult".
Well, this discussion is obviously going nowhere, because I haven't played B&S but I can't see how it differs from the formula every other half-decent Asian MMO uses, and you guys haven't played WoW. I feel like the only reason it's being compared to WoW is because it's the biggest MMO, though.
WoW isn't terribly difficult, as it was designed to be much more appealing to casual players than Everquest, but it's not just getting "12 year olds to cooperate", either. The current heroic content hasn't been completely cleared by very many guilds. One of the harder fights:

This is a test of execution. Anyway, I'm not saying it's hard, I'm saying it's not a mindless free-for-all and the notion that enemies having attack patterns in B&S is something innovative is ridiculous.
