Just to add something on top of what E6 said:
Manu, make sure you max out Q ASAP. That thing will be your ticket to everything Karthus. You can farm with that skill (last hitting is always good; you can get some mana back with the passive from your E skill), AND you can harass whoever you're laning against with that skill. But of course, it does take some practice to skillshot correctly.
Its CD is very, very miniscule, (<1 seconds) so you can pretty much spam it. Mana cost shouldn't be a problem when you use that skill, unless you decide to spam it 24/7.
And of course, when your ult comes up, you have to know when to use it. Its not fun when you nuke someone and they have some health left after that. Karthus's Requiem is meant to terrorize those who have very, very low health.
