The way I understand this, you can't register a .xxx address without going through this ICM Registry.
No one is forcing a porn site to be named .xxx. There will still be .com or .co.ru or w/e. But those that want the "trustworthy" tag of being an official .xxx, they will pay ICM to vet them by giving them such a name.
As I said above, it remains to be seen just how many of them will opt in.
