There were some slight inconsistencies with this formula. The one that comes to mind is Scarabs possibly having up to 39 w.atk clean (34 average) though I can't recall ever finding one like that myself.
Anyway, yeah, it's supposedly like that up until +-5 from average stats, before BB the most you could get outside gachapon. After BB, I dunno. I check my items' stats before dropping/NPCing them but I haven't seen anything amiss on a stat that counts.
And I don't think Gacha can add as much stats as you said, though there's a strong lack of info on items with 10~40 stats.
PACs 2 watk average, max +2 (4 atk)
PGCs 3 watk average, max +2 (5 atk)
PuACs 7 matk average, max +2 (9 matk) etc.
Higher level weapons, max +7 (eg. Sky Skis, 94 atk, max 101?)
Post-Item Maker it's impossible to tell what drops or comes from gach on weapons that can be makered - they forge up to +11 (with adv. bc/diamond and good luck).
