Its never a Win-Win situation to be honest.
I would honestly choose to save my wife and hide the truth to give her time to recover from the surgery and the biological changes that comes with pregnancy so when she finally knows the truth shes in top notch condition to deal with the horrible event thus the Emotional scarring would be FAR less. What happens after that depends on the personality of my wife.
But if you choose to save her then that its most likely not her last wish (NOTE in case the surgery its 100% success rate) and if you chose to save the child then you are mostly grating her last wish is it not?
Whatever option you choose its not easy because you are killing someone, and thats not pretty is it?

I can understand you not wanting your husband to go against your wish, but him actually choosing the better option for you against his own wish is something I would not take for granted, nor want my girl to.
