the dorans blade builds are for people who merely know how to play ranged physical dps and dont necessarily main them. its just safer. but if you know what you can and cant do well, the extra survivability is unnecessary and opens you up to other strategies. i rarely die within the first 10 minutes of a game. after that, the extra survivability is moot because by then you start getting into hard ganks and teamfights -- if youre focused youre dead whether you have 3 dorans blades or a bf sword.
Starting with shoes is only fine on kog'maw, since he doesn't focus on AD.
Starting with doran's blade is ideal no matter what. It's better in every way except movespeed, and movespeed 1 won't do anything special for you at level 1~9 that is really needed.
EDIT: Oh and using boots as a way to avoid death in teamfights is a bad excuse for starting with boots. You get boots regardless of your starting item(s), while you're building your IE (assuming generic ranged carry) or after.
