Sorry but i got to disagree with buying a red pot at the start of the game on fiddles, a blue pot is far better.
Red pot: Gives you 200 hp
Blue pot: Let's you drain an extra time which can heal you for way more than 200.
And Merc threads is in most situations way better on fiddle, you want to be able to move around as freely as possible and have the lowest possible duration of CC on you.
And i've never been a fan of haunting guise :l
Also if you want more HP on fiddles than RoA, go rylai's. Rylais + his ulti/drain=devastating.
And zhonya hat is a core item on fiddles.
Cast ulti into enemies, for example at their tower that they're deffing while your allies are infront of the tower about to engage, you ulti splitting the enemies up and then zhonya's hat while your allies engage. So yea, gotta agree 100% on that one
