Imagine that the gate you make in the Real World has coordinates. The game tries to make a the second gate in the Nether using those coordinates. But the game defines it as unfit. "Unfit" could be anything from being over a sea of lava to being inside of a wall. The game then moves it to a 'fit' place. When you re-enter that portal, it creates a portal with the same coordinates in the Real World.
End result: Two portals in the Real world that both lead to the same portal in the Nether.
Now I have to wonder... if you hacked your Nether file to be solid block, where would your Nether portal spawn?
