I believe this is just your client's RNG and the server's getting out of sync.
I've had several times where every time I saw a 1hko from my ring on my screen, the NEXT attack (or the one after on occasion) would kill anything (this was on my (former) hermit so I could tell the order in which monsters were being hit)
