9/6/6 is probably the most common, followed by 9/9/6 and 9/6/9, chance of that occuring seems to be in the low 2% ish, it's hard to tell since a lot of equip gives "crap" properties that isn't stat related. In regards to getting primary higher than your existing rarity, if you roll a +3% str on an htp, there's a chance to obtain +6% or even +9%, I'm guessing each rank is around 5%, meaning if you get +3% str, there's a 2% chance to get +6% str instead, and 0.04% of getting +9 str%. (Figures are of course just my estimates) Basically, I think how it works is it rolls for the potential option, then it checks the lowest rarity bonus it may have, then it checks for chance of obtaining the higher ranked bonus.
Rare item (level 100)

First line is primary, so you're guaranteed +3% str, that's it, it doesn't even give you possible higher ranked stats.
Second line is secondary, so you're guaranteed at least 50 weapon def, with a certain % chance for getting 100 weapon def (tier 2)
Third line is secondary, so you're guaranteed at least +2% mp, with a certain % chance of getting +4% mp, or even +6% mp. (tier 2 and 3)
Even though the bottom can obtain hyper body, because it's impossible to roll for that iteration, you will not see it at any state but at unique rank since its prefix is 03, not 01.
From rare -> epic, chance is around 4%, from epic -> unique, chance is around 2%. Just from personal experience from cubing.
