Threaten hits 6 targets, and like every other warrior skill, it probably chooses to hit the stuff that spawned first (rather than based on how close it is).
Unless all 8 bodyparts of HT were in range it would debuff whichever you wanted though. Don't think it'll hit the heads from the ground floor.
