I noticed this on my Marksman all the time too. God it was horible.

This always happened to me when I tried using blizzard RIGHT after dragon's breath. This only happened when the monster was so close to me, that using DB caused the guard to turn around and thus enter my "mortal blow" zone (MB glitch, anyone?) and when I try using blizzard, it tries using MB instead, thus causing the DC.
That's what I always noticed, who knows if it's right.

EDIT: Just rewatched the video, he was no where near MB zone when he DCed. I guess it just has to do with using blizzard too fast after you use DB.
