Normal/Rare/Epic/Unique Items

So, question here. When this came out over there, were all pre-existing items marked as "normal", or were some randomly labeled as "unidentified"? What I mean to say is, are these ranks something default with the items just like stats that are randomly created for drops, which would apply to all or only new items that drop; or are all existing and dropping items ranked "normal" until whatever that scroll is used on them to give them the "improved" ranks.
