A good program for this would be RoboForm. You enter a fake email, name, address (you can just google a legitimate one), title (Mr./Mrs./Ms./etc), phone number (also easily googlable something like Safeway or Target's phone number should suffice), and....I think that's all. When you get to the survey section just click auto-fill and it fills everything for you. It can do checkboxes and stuff, but I don't know how to configure it.
You can find the program at Cnet. Oh you can also set income, where you work, what kinda job you have, stuff that surveys tend to ask. I used to use this for Prizerebel, and all those other sites.
