I set up my keyboard for comfortability.
Rest my hand naturally on it, see where the fingers go (C F Y J) - I chose these partly because of the little bumps on F and J to help me keep my hand centered. Big hands small laptop, it's probably ok to go ASDF or something on a fullsize.
Those 4 keys get my primary training buttons (jump, loot, attack 1, attack 2). Then I see what's comfortable near them and add any useful buffs there. for example G has mana pots because my laptop keyboard does not mesh well with the quick slots. I usually put booster on X.
Most classes don't have more than 2 attacks so it's really not an issue. My brawler has H and K for attacks, as well as possibly in future X and V. I guess I use K for Rush on my Paladin, which is kinda the 3rd attack.
In terms of comfort the worst for me is the spacebar. It cramps up a lot and I'm better off not using my thumb at all during training.
I should note that I don't use QWERTY so my Y key is not actually the same as NPC chat "yes". You'd probably want to move everything over a key or something to compensate, because having the NPC chat on "yes" is really useful.
