If a monster is poisoned three times already, can the duration be maintained indefinitely or will the poison have to run out before being able to add another poison?
You can poison a monster more than 3 times. I put on weak gear for the purpose of trying to figure this out and I've been attacking vikings - I was able to poison it 4 times. So I know that it's only 3 poisons at any given time for sure. Doing some further testing, will report back.
EDIT: It looks like you must wait for the current poison to run out (I hit ~6k 3 times and on the Triple Throw immediately after, I hit ~10k 3 times - with venom damage, of course).

Adding venom hits does extend the duration of the previous hits to the new damage, I think. That seems to be the case as I've seen 2.4k, 2.4k, ~7k, ~7k, ~7k in succession with a break afterward.
