Something to note - if the ult'r casts BEFORE you cast HH, even if HH's animation finishes first, if the monsters would've been killed by the ult, your HH will never hit them. This happens a lot because of the long skill animation, but the damage being calculated instantly. Doesn't sound like your problem, but ya never know.
