Just to point out for those getting on their soapboxes, a guild member of mine WAS mistakenly perm-banned recently. He was ccing through WS and a GM saw him on the map at the same time as a hacker and permanently banned him. Even though you only had to check that:
1) They weren't partied
2) The vacced mobs were not going where he was
3) He cced about 2 seconds later
And he had to go through a whole lot of crap to prove his innocence and turn his banning.
