In Kradia, there was absolutely no-one at the Zenumist waiting room. Everybody (which was never more than about a dozen people, but yeah) was at Alcadno. Which didn't save us the run to the other end, just in case there was some haster sitting there.

That's sort of a chicken-and-egg answer, though. They could have had the PQ only run to level 80, and removed the level 90 Lord Pirate hat from the game.
Or have it given in some level 90 personal quest, something along the lines of "The evil Lord Pirate is back! We have no time to gather a team of heros to fight him, please go do it yourself!" and then you kill some new, "area boss" incarnation of this guy, for a bunch of fame, exp, and the level 90 hat.
