Unless you are thinking of lynching the doctor, I don't see why we should ask him to come out. That would only invite the Mafia to kill him, or one of the cops. If our 3 most important roles are out in the open, it would only be a short matter of time before the Mafia eliminates them. And if they get the doctor, the cops will die shortly afterwards.
But I do agree, we should probably decide if inactives are important or not. If we kill them off, we could possibly get a mafia member that has been staying silent, worst case we kill off a townapple that was not contributing.
The problem with asking the doctor to come out is that an inactive could claim to be a doctor and was staying silent to avoid the mafia looking at them. We can't really prove a doctor, unlike a cop. Claiming to be the doctor gives immunity to lynching, but if a mafia member claims it first it would be harder to prove. At least in the cops case, you can lynch and see if he was right.
