Kirito dual-wielded in the Aincrad arc, once...no, twice(?) in Fairy Dance, then dual-wields one more time in Mother's Rosario. Only difference is that the Dual Blades skillset only existed in the Aincrad arc. Kirito only really dual-wields when he gets serious and thinks he needs it to win. Of note, he actually lost to a group of unnamed players while dual-wielding in Mother's Rosario; he lost off-screen (got ganged up on lol), because he wasn't the main character that volume.
And yeah, he can't dual-wield in Underworld. At least not with high priority swords. He could barely even move carrying the Sword of Blue Rose, even with his Black Sword sheathed.
EDIT: DAMN is Saint Hale hard. That was the first time I've ever come close to dying in a solo dungeon. ._.
The final boss of Wrathborne though, was laughably easy.
