I got permabanned once. Maybe it was because I went to attack a hacker's mobs for quite a period of time because I was bored. I got unbanned after the GM said I was involved in "suspicious activities".
After a while I got tradebanned because items I put in my shop duped back into my inventory because of a crash hacker, and they unbanned me again after the GM said I was involved in "suspicious activities".
I think they have a difference between "suspicious activies" and "malicious activities". When they say "malicious", it means they have concrete proof you were hacking.
