We dont get hit at all, even in ht, czak, I use 0 hp pot since the patch. Someone said they afk in hoh for 3h and didn't get hit at least once.
It only works for atk, m atk and touch. Poison, falling rocks, dr/other stuff does dmg.

Also, you will likely be taking damage from hard mode hillia, Chaos PinkBean, and magnus since they all max max accuracy.

pretty sure they're scripted to hit, max accuracy would still get stopped by skills

But they still miss at times if you have max avoid. But it surely happens more often than with 755 accuracy.
