Generally, I try to choose the one that will help me out the best in the long run.

Gen 1: Venusaur. This guy is a freaking tank with sleep powder, leech seed, and his overall good defenses. If you aren't ready for this guy, you'll be tearing your hair out.
Gen 2: Feraligatr. He just looks the coolest to me. Not really a fan of fire types and Meganium is just plain silly.
Gen 3: Blaziken. Gen 3 had one of the coolest pokes, but Blaziken is just plain rape in battle. Great special attack and attack to hit from both sides of the spectrum.
Gen 4: Torterra. Who wouldn't want a grass and ground type pokemon as a starter? Just use Rock Polish to beef up his horrible speed and you've got a monster in your hands.

Looks. I usually go for the one with the cutest/coolest looking final stage evolution.

Gen 1: Venusaur. I used to go for Charizard, but recently (FR/LG) I've been choosing Bulbasaur 'cos it's supah kawai.
Gen II: Tough one, because they're all fugly. Most of the time it's Typhlosion, but sometimes I go for Feraligatr.
Gen III: Blaziken. Dusk, I have no idea why you think it's ugly. I just think a fighting chicken is awesome. Swampert is great in battle, Sceptile's cooler, but I just like Blaziken.
Gen IV: Torterra. Awesome, badass, (somewhat) great in battle. Infernape's just a crazy monkey, and I hate it that it has the same type combo as Blaziken. Piplup's cute, but the other 2 evolutions are fugly.

I thought a fighting chicken would be awesome too. I picked Torchic when I tried out Ruby (never finished that game). Blaziken doesn't look like a chicken at all. It looks like a retard wearing a vest, long pants, and a goddamn mask for a head. And don't get me started on the "hair".

I always start with fire. Mainly because fire does the best in game for the story because of the offense based stat layout and nice type coverage. Water and grass always seem to have more of a "take hits and last long in your battles" stat build, but if you're going around 1 or 2 hitting everything with a bag full of potions and revives lasting long isn't really that needed. That and I usually build my final team around the 3rd or 4th gym, and sometimes a good fire type isn't present at that point, or I don't like the fire type that is available(while I like the water/grass types there are most of the time).

Best looking, normally. Sometimes stats, however.

ie. In Soul Silver I chose Cyndaquil, because I hate Chikorita and I don't like Totodile as a water type, Squirtle, because I wanted a water type and I didn't want Totodile nor Mudkip (I would've normally chosen Bulbasaur otherwise, because it's beast.), and Treecko, just because it's beast looking and I don't like Mudkip nor Torchic.
In 4th Gen, I would choose Turtwig because it's beast as well and I like grass types.
