Here's what I have so far, it's pretty crude though.

Swamp Bog is the first map. Right portal leads to the beginning, left portal leads to the far right of Crossroads 1. (The numbers don't show; it's like the haunted house duplicates, they just help you think.)
Crossroads 1 has four portals on ground level, plus several more up above (which can't be reached except by other portals). From left to right, these lead to:
Haunted Hill - Twisted Paths 2 - Twisted Paths 1 - Swamp Bog
Twisted Paths 1 is a flat map with 5 portals in a row, and from left to right these lead to:
unknown - Twisted Paths [?] - Crossroads 1 - Twisted Paths 2 - Phantom Road
Twisted Paths 2 has an identical layouts and the portals lead:
unknown - unknown - Crossroads 1 - Twisted Paths 1 - unknown This is from what I remember; I didn't actually write it down, so it may be off.
Haunted Hill has 2 portals at ground level, a hidden portal that looks like a signpost, and a third portal at the top of the map. I wasn't able to reach the top portal (you need Teleport) but the signpost leads to Creeping Evil (which is a dead end map with a nice spawn of Leprechauns). The ground portals are:
Bent Tree - Crossroads 1
As for Bent Tree, it has 5 portals (refer to extractions thread). Far right goes to Haunted Hill, far left goes back to the beginning, and upper left goes to Dead Man's Gorge which is the Raven Ninjas' camp. The other two, I dunno.
Yeah. That's pretty much all I've found so far, other than random illogical portal-going. Hopefully though... someone more capable than me can do (or maybe even has already done) much better.

Anyway, I'm going back in (I dc'd lol, that's why I'm here in the first place and not exploring some more) and using another way of recording these portals that makes me go, 'doh, why didn't I think of that earlier'.

The most confusing thing for me is that sometimes a portal is different going through one way than another.
For example, if you go to the 1st map, take the center portal, you end up at a crossroads. Go one left, and it takes you to Swamp Bog. However, go back through that same portal, and you're at a different crossroads (however, this last portal still leads to Swamp Bog).
Definitely worth marking down portal destinations in both directions if you're drawing a map, and don't assume Crossroads are the same just because they look like they are.
I tried going through portal after portal, in Twisted Paths, and I thought if I hit a Crossroads, then I did something wrong. Also, if I hit a map that has another name, then I'm going in the right direction.
It took me a while to figure out that all of them except Forgotten Path are just there to confuse you.

I decided to do it Hidden-Street style, give each portal on each map a letter and list where it goes. The Twisted Paths and Crossroads are definitely the most puzzling...
One way to make sure whether a portal is two-way is to drop a feather before entering. It puzzled me for awhile.
I made a little progress on the duplicate maps, but it's hard to check whether one map is the same as another...
Sometimes I wish I could just drop a mule char into a map and make him stay on the map without actually running the game on another computer. Times 5.
Hmmmm I've drawn a full A4 paper full of maps with lines now, and it semi-works, but some teleports can't teleport you backwards to that place you came from since they're 1 way only...
So creating a total map WILL be very difficult, since you would have to include 1-way teleports...
