how to earn that new shoulder equip (+5 all stats, with 3 slots) ? is it a one time quest thing? or from larson or what?

Do stuff -> get coins for medals -> get a certain medal/rank of medal and it unlocks the ability to buy shoulder/scrolls -> buy f'uck tons -> ??? -> FAILURE.

After you've completed the quest chain, repeat a quest or two until you have around 250 coins or something. Trade those coins in to the guy with the off-white hair for ranked medals. Once you have the proper rank of medal, talk to the wooden box thing and buy the shoulder pad for 7.5m. You can buy as many as you wish.

Because the scrolls only add two attack and Chaos Scrolls can add 5?

My bad, i tend to skip over the "Consume" part of the extractions do to it usually being full of placeholders and nonsensical stuff. (to me at least)

If you're over level 110, it can easily be done in just a couple of hours.

I did the entire quest chain (or at least as far as we can go in tespia), and did all the repeats, and I'm still 15 coins short for the scrolls.

Yeah, as musclewizard said, not until tomorrow. So if you do every single quest, you can get enough to purchase the shoulderpad on first day, but you'll have to wait til second day to get the scrolls.
