How to combine 2 items to one?

I've heard that you can combine 2 items to one, but i don't know where can i do this ><"

The alchemist profession allows you to fuse two of the same type of equipment (armor/weapons) to form a new one of the same type with a chance of potential.
IE: Two work gloves => 1 work glove with chance of potential. If the two items to be fused have potential then the new one haves "100%" chance of getting potential. Higher alchemy level = able to fuse higher level items.
