A couple times I've noticed my Crusader Codex has no potential on it - the set's still shown as selected, but I have to switch to another one and back to get the bonuses working (it's the Leafre set so it's pretty obvious if +1 all skills is missing).

does it to me every time i log out of the character. Ugh this patch has a few annoying glitches.

I'm losing my Codex effect from changing channels too, I believe.

Mine resets each time I log on, so it sucks when you d/c or crash.

Lol I just tried to test this to see if it was happening to me as well. Apparently changing channels can dc you. Fun times.
