Does this increase the cap of 4th job skills like CO does?

i dont think so...

i saw that Fiel said CO is the only skill that "break" the max level of skills.

No. "Potential" on the Monster Book works the same way as potential on every other skill, minus the ability to Miracle Cube it. It'll only give +1 to all skills if those skills aren't maxed.

You get all of the bonuses from whichever set you choose have active. As for leveling the Codex, you must complete sets before you can gain any points toward leveling your book. I don't know the thresholds though.
