I've experienced her 1 hitting me if I stand to close to her when finishing off a v1 summon.
It's like as if I get hit by DR out of nowhere.

the only real bug i know of, is, if she does genesis, AND sleeps before all the attacks come out;
if you are standing where a genesis volcano was before the sleep, you will instant die, when she un-sleeps.

I think she dc the entire pt somtimes
, maybe it's the glich about dcing people when moving to the map with some mobs , that's why i don't try empress anymore we got dc a lot of times.Edit: i'm not really sure what dc my pt sometimes(if it's really the map that can dc you).
@kayeyearekay sometimes i die when she summon the v1 i think when i'm too close.
