Empress shoe: never equipped, hammered twice, scrolled with one Incredible Chaos scroll.

Is it the hammers? Or does it count never-equipped Empress as being scissored? Or is there something else I'm missing?

I think you have to use up the hammered slots; try passing a random shoe/armor scroll, and then use the innocence scroll.
