Asking for a little help here, since i don't have much time to investigate this... Does anyone well versed in SQL can tell me what are the sentences i need to make this DB?

Its pretty simple stuff i know, but i don't know if i have the time to look and understand it at the same time how to do it (Since i skipped those classes and my friends aren't exactly helpful when i ask them for info). Any help its greatly appreciated.
