EDIT; From looking at that video on Moetron, it looks like ZUN kept the point scheme from UFO and the Spell Card system from Fairy Wars and added some more new stuff.
Huge difficulty spike at the stage 3 bosses, as always. Miyako Yoshkia's (Stage 3 boss) spell cards are extremely difficult to pass. Level 1 and Level 2 shots hardly do any damage and if you die during her spell cards, she recovers life.

Miyako's first spell is retarded.
She heals faster than you can kill her. If you just shoot constantly, you'll just heal her more. As youmu, if you charge shot to do damage, she'll heal like 1/4 of her life everytime you slash.
I can't imagine doing this from a range unless you very selectively shoot or you go up to it and shotgun.

Yeah, I need to find some way to actually beat that card. You actually get punished for killing the familiars that are right next to her and to make matters worse, the spell card has an annoyingly short time limit.

So far, I like it.
Kyouko and Miyako are certainly welcome additions as new Touhous.
Kyouko's animations during the fight where she sticks out her tongue, closes her eyes, and makes that cute pose with her hand are so adorable. One thing I have to really praise about Kyouko is the "gimmick" she uses, those walls she creates to reflect her bullets off of. It's really unique in my opinion, and it has you think fast on your feet by having to scan both sides to figure out where the bullets are going to go after she takes out the walls.
Miyako also has a really cute design in addition to being the now second Chinese Touhou. Her zombie walk pose is kind of cute. However, I found her spellcards to be kind of dull. She does have a pretty neat (although somewhat cheap) twist, though- that makes her stand out as pretty unique. Since I bring that up, I'll address this post:

Possible that you are inflicting damage, but she's healing fast enough to compensate for the damage you do. When you die, you release spirits; which she then proceeds to suck up. I've had a really hard time trying to pass her spellcards because of the fact she always regenerates.
Also, as for our playable characters:
-Reimu and Marisa are pretty much like they usually are.
-Sanae has a really, really nice wideshot. Her shot starts out as wide as Marisa's full power shot, if not a little more wider than it. Sanae's my personal favorite this time.
-Youmu's charge slash demolishes everything (except for Miyako, asdqbojoif). I want to try to play her more.
