He can change the hormones in his body to various degrees, including stuff like giving someone everlasting adrenaline for the day, to completely changing gender (yes they are real and it's NOT a trap). If I didn't know the truth about him when I saw the second pic, I may have actually considered "I'd tap that". Curse you Ivankov and your ambiguous nature!
On the other hand, this is what makes One Piece so awesome. There is absolutely no limit to what can exist in this universe. We already have a rubber hero (Luffy) and a walking black hole (Blackbeard). Why not also have a true gender bender that is also a miracle healer?
