First thing I died to was the fire goblin thing since I kept getting burned.
You upgrade at any blacksmith, +1 to +8 only cost bronze coins (in cbt). You're supposed to be able to convert frags into whole Onyxs but you needed an item not in the game during cbt.

Actually, you just need three Onyx Fragments for a Crude Onyx, but you needed to Craft them, and Crafting wasn't in CBT.
I got all my Crude Onyx from Sanctuary Nexus. Easy/Normal mode runs got me 2-3 per run.

Some of the lower level plates (lvl 10+ ones) could use just the fragments. I had a couple on my archer before the CBT ended.
And you could get at least 1 crude onyx from a quest. I don't remember which one but it wasn't that hard.
