I got to Utero 1 by luck yesterday due to getting absolutely insane luck with items. xD
Most of the time I get squished around the caves though. I'm doing better than I did in the first game though, probably because the controls are easier to use.

Managed to get Guppy on Caves 2 along with the shop item that increases size/damage of flies/spiders.
Proceeded to wreck everything including Satan.

Ludovico technique (spelling?) + brimstone is the most OP combo I've come across, christ.

And then I got spoon bender on top too, so the beam started homing when it was close to enemies.

Technology 1 is so disappointing now. In the original you could get all sorts of massive, cool looking lasers. Now it's just this tiny little laser no matter what items you have.

I had an absolutely hilarious run earlier. I'm not quite sure what the full combination of items were but I ended up basically 1 shotting every room because a foot would squish people, gemini would attack stuff and I had brimstone. Absolutely insane.
I even 1 shot Mom which made me die laughing. Gotta love when this game throws a ridiculous run at you after some bad ones. xD

Triple Shot, Pee Shots, Brimstone + other Tear up effects. My run was over in less than 20min so I went to pick up another item for me and kill bosses, all of them went down in a single shot, dying in pairs.

They confirmed in an interview that they have already started work on another expansion. (and that they are incredibly pissed that people datamined the game to find the secret stuff, since it was supposed to be a community project to discover it and unlock it, they spent months planning it and hiding it, and it was datamined in under a week)

Found an awesome combo, Bouncing Tears + Homing Shots. The tears bounce off the enemy and then turn right around and hit the enemy again. I beat Isaac really easily with this set up because the bouncing tears just kept on hitting, even while I was running around to avoid his attacks.
Then again, homing shots + anything is awesome.

I'm really surprised at how good Chocolate Milk is now. In the original I never even picked the item up because it was that bad.
I just had a run that started out with Chocolate Milk + Piercing Shots + Pentagram (XL Floor) and it ended up going all the way through to Mega Satan (lol, Dad's Key opens Mega Satan's door) and I'm pretty sure Chocolate Milk is what carried me through. Although I was able to pick up Scythes and some other decent damage ups along the way so of course those helped as well.
I also discovered God Rooms are now twice as good as they were in the original since you can kill the angels and then reroll the key pieces into other god items. Although this means missing out on the Mega Satan fight, in most cases.

I swear they have lowered the drop rate of Soul Hearts from the original game. playing as ??? is torture, I've gone 3 or 4 floors clearing every room and never finding a single soul heart. It makes every hit that much more frustrating because the health is so hard to get back.
I've had runs where I have gotten absolutely god-like combos of items offensively, but finding not a single soul heart and falling to the first or second boss because the game will throw me up against a bullshit boss like Dingle or Haunt which are hard to avoid getting hit on.

What I did with Blue Baby was only go until I got to Mom's Foot and if I didn't find some sort of item to generate soul hearts by then, I would just reset (Book of Revelation, Dark Bum, The Relic, Mitre, Crystal Ball, etc.).

That's why I usually mix my Blue Baby runs--well pretty much all my runs--with both Soul Hearts and the new Black Hearts. Items that grant Black Hearts are a lot easier to find, and honestly they're a lot better to pick-up anyways.
Also: I don't know if you knew this or not, but the skulls in the later areas have a chance to drop Black Hearts, amongst other things, when destroyed. Likewise, the Blue/Purple Flames can be destroyed by bombs and they have a 50% chance of dropping Soul Hearts.

My favorite way of doing tough challenges: finding a dice room to reroll all my items into a better combo.
